Ocean Cure Adaptive Equipment Waiver
By reserving this equipment (beach wheelchair or beach walker), I agree to the following:
1. I accept full responsibility for the safe use and return of the beach wheelchair or beach walker and any associated equipment provided by Ocean Cure.
2. I understand that Ocean Cure, its volunteers, directors, and agents are not liable for any injury, accident, loss, or damage that occurs during use of this equipment.
3. I will notify Ocean Cure when I am finished using the equipment and will ensure it is available for pickup or return at the agreed time.
4. I will store the equipment in a safe, secure location and will not leave it unattended where it could be damaged, lost, or stolen.
5. Failure to return equipment may result in responsibility for replacement costs and ineligibility for future reservations.
6. Ocean Cure reserves the right to cancel or modify any reservation based on equipment availability.
